This form must be completed as part of any unsolicited proposal submitted to the DC Office of Public-Private Partnerships (DC OP3).  It will be made public along with all other non-confidential aspects of an unsolicited proposal as part of a fair, open and competitive procurement process if the proposal is determined to be favorable.
